By e-signing this form with your name and email, you give permission to Dr. Maeve Kane, Department of History, University at Albany, to record and quote from your interview.

The form below asks for your consent for our interview to be recorded or not. Regardless of whether or not the interview is recorded, you may at any time withdraw consent for me to discuss any part of our interview in print. If, at any time before the publication of my book (anticipated in 2025) you want to redact any or all of your interview with me, please let me know by email at [email protected].

If you consent to have your interview recorded:

The recording of your interview will be used only for educational and research purposes, limited to the scope of my book project only.  I will store the recording and backups on my personal hardware; I will not archive or transmit the recording of your interview to any other person or institution without your further written permission.

If you consent to recording the interview, your e-signature releases all copyright, title, and interest in the recording and transcript. This allows me to allow me to quote from your interview for the eventual book.  You retain all other rights and the right to quote from, redact, or otherwise use the recording and transcript of your own interview.

After our interview, I will send a link for the recording and transcript of our interview to the email you provide here. The transcript and recording will be password protected online, and only visible to you and me.

If you prefer to be interviewed without a recording:

I will take notes during our interview and will reference paraphrases from these notes for the eventual book. Your e-signature indicates your consent for me to describe our conversation.
